Lymphoedema Management
Reducing Swelling & Improving Range Of Movement
Phase 1
The Intensive Stage
The intensive phase comprises a series of Manual Lymphatic Drainage (MLD) sessions and compression bandaging. Its goal is to manage oedema by redirecting congested lymph to a well-draining area, thereby reducing swelling and enhancing skin condition and mobility gradually. This phase entails daily sessions spanning 3 to 5 days, with 2 to 3 appointments per day initially, followed by weekly appointments until the oedema has reduced as much as it can. Subsequently, you will transition to Phase 2 Maintenance Stage.
MANUAL LYMPHATIC DRAINAGE
To help remove excess fluid from the affected area. MLD is noted to be very effective when combined with compression bandaging.
COMPRESSION THERAPY
Multi-layer bandaging helps maintain the reduction achieved after MLD. It increases lymph and venous return and helps reduce changes in the tissue associated with lymphoedema (fibrosis). The foam under the bandage helps increase fluid movement and reduces bandage slipping.
SKIN CARE
Good skin care is essential to avoid infections. It involves the use of emollients, noting any cuts or abrasions and taking action at the first sign of infection.
EXERCISES
Light exercises play a key part. Special simple exercises with the compression on helps circulate lymph out of the swollen area (further reducing the limb). Breathing exercises are also key as the main thoracic duct vessel runs upwards through the chest cavity to carry lymph back to the heart.
Phase 2
The Maintenance Stage
The aim is to maintain the results from the Intensive stage. Follow-up MLD treatment is carried out weekly or monthly depending on the individual case.
Custom compression sleeves / garments are worn daily to control the oedema.
Weight control is key to avoid fat cells compressing the lymphatic system and reducing the flow.
Exercising is preformed wearing the garments and they should be replaced every 4–6 months.
When is DLT not suitable?
- Active cancer
- Cerdiac oedema
- Deep vein thrombosis (DVT)
- Active infection
What is the difference between MLD & DLT?
MLD treatment alone is the hands-on technique to manually drain lymph fluid away from the swollen site area.
DLT is an intensive treatment that combines MLD with bandages / compression garments, exercise and skin care.
DLT is done regularly over a certain amount of time to drain as much lymphatic fluid from the swollen area.
What is the aim of DLT?
- Improve lymph flow
- Loosen hard, fibrotic tissue
- Improve mobility for the muscle and joint of the swollen area
- Help avoid infection or pain
